The 2025 AHA/ACC Guideline on the Management of Hypertension replaces the 2017 document and introduces a modern, risk-based framework. Central to this update is the PREVENT™ risk equation, which expands age applicability, integrates kidney function and social risk, and demonstrates superior calibration in diverse populations.
The scope covers prevention, detection, evaluation, and treatment of hypertension in adults without established cardiovascular disease. Recommendations draw on a comprehensive evidence review, including randomized trials, registries, and cohort studies, with independent review committees addressing priority clinical questions.
The treatment goal remains <130/80 mm Hg. For stage 1 hypertension, lifestyle intervention is first-line, with drug therapy advised if blood pressure stays ≥130/80 mm Hg after 3–6 months. For stage 2, fixed-dose single-pill combinations are prioritized to accelerate control and adherence.
The guideline underscores team-based care, validated home monitoring, and community-level screening as strategies to improve outcomes. Developed with and endorsed by 11 national professional societies, it is published in JACC, Circulation, and Hypertension.
