Young-onset type 2 diabetes mellitus (YT2DM) is increasingly recognized as a more aggressive metabolic phenotype than older-onset type 2 diabetes mellitus (OT2DM), with earlier progression of complications and greater long-term disease burden. A cohort study published in Diabetes Research and Clinical Practice evaluated whether diabetes remission rates after metabolic surgery (MS) differed between adults with YT2DM and OT2DM.

The analysis included adults with obesity and T2DM who underwent MS at an Australian tertiary hospital between June 2018 and May 2024. YT2DM was defined as diabetes diagnosed before 40 years of age. The primary outcome was complete diabetes remission at 12 months, defined as glycated hemoglobin (HbA1c) <6.0% and at least 3 months without glucose-lowering medications.

Findings

  • Complete diabetes remission at 12 months occurred in 38.1% of patients with YT2DM compared with 75.0% of patients with OT2DM (P = 0.002).
  • The study included 44 patients with YT2DM and 36 patients with OT2DM.
  • Differences in remission rates were no longer statistically significant after adjustment for baseline variables.
  • Baseline HbA1c and duration of T2DM appeared to contribute to the observed differences in remission outcomes.

Adults with YT2DM achieved lower rates of complete diabetes remission after MS compared with those with OT2DM in this cohort. Baseline glycemic burden and diabetes duration appeared to influence remission outcomes, supporting consideration of earlier obesity treatment strategies in YT2DM.
