Doctors in Japan have found a simple probiotic that brings real relief to type 2 diabetes patients also battling chronic kidney disease and stubborn constipation. Constipation hits hard in these groups, slowing bowels and hurting daily life, but treatment options stay slim.
This multicenter trial published in the Journal of Diabetes Investigation put 30 such patients on Bifidobacterium bifidum G9-1 for 12 weeks in an open-label setup. They tracked stool shape using the Bristol Stool Scale, bathroom trips, gut symptoms, and life quality. Blood sugar and kidney markers got close watch too. The goal was to see if these gut-friendly bacteria could fix stools without side effects.
Stool Shape Turns Normal for Many
At the start, just 3.3% or one patient had ideal stool form—Bristol scores from 3.5 to 4.5. By week 12, that jumped to 44.8% or 13 patients, a big shift that cleared tests. Median Bristol scores rose 0.6 points by week six and 0.5 by week 12, both highly significant.
More Trips and Less Gut Pain
Patients went to the bathroom more often too, up 0.2 times per day on average by week six. Gut complaint scores dropped sharply for constipation, belly pain, and upset stomach. Straining, leftover feeling, bloating, and discomfort all eased up a lot.
Kidney and Sugar Markers Hold Steady
Good news came on the kidney front—urine albumin fell significantly by week six, while key kidney filter rate stayed even. Blood sugar marker HbA1c showed no change, proving safety.
Safe Gut Fix for Tough Cases
These bacteria work by balancing gut bugs, softening stools, and calming irritation without drugs that might stress kidneys or sugar control. For endocrinologists and nephrologists, it offers an easy add-on to ease a common problem.

Key highlights
  • Bifidobacterium bifidum G9-1 raises normal stool form proportion from 3.3% at baseline to 44.8% at week 12 (P=0.002).
  • Median Bristol Stool Scale score improves by 0.6 at week 6 and 0.5 at week 12 (both P<0.001)
  • Defecation frequency increases significantly by 0.2 times/day from baseline to week 6 (P=0.026).
  • Gastrointestinal symptoms like straining, bloating, and abdominal pain decrease markedly on rating scales.
  • Treatment lowers urinary albumin at week 6 without changing HbA1c or eGFR, confirming safety in T2DM-CKD patients.
