The ADVANTAGE AF trial highlights the safety and sustained rhythm control of pulsed field ablation in patients with persistent atrial fibrillation, supported by continuous rhythm monitoring.
The Phase 2 results of the ADVANTAGE AF study were published in the Circulation. This study incorporated continuous rhythm monitoring using insertable cardiac monitors.

A total of 255 patients (average age 66.7 years, 29% women) underwent pulmonary vein isolation (PVI) combined with posterior wall ablation using the FARAPULSE PFA system. One hundred forty-one patients underwent cavotricuspid isthmus ablation for typical atrial flutter, achieving a high success rate (98.6% bidirectional block) with no major complications. The procedure achieved 99.6% acute success for PVI and 100% for posterior wall ablation, with an average procedure time of 105 minutes.

At one year, freedom from atrial arrhythmia was 73.4%, with only 2.4% experiencing adverse events. While 52% of patients were free from atrial arrhythmias lasting 30 seconds or longer, the majority (94%) had no episodes lasting longer than 24 hours. Atrial arrhythmia burden greater than 0.1% or episodes lasting longer than one hour were associated with increased healthcare utilization.

Key highlights
  • PFA achieved high acute success: (99.6% for PVI and 100% for posterior wall ablation).
  • Cavotricuspid isthmus ablation was effective (98.6% bidirectional block) and safe with nitroglycerin prophylaxis.
  • At one year, freedom from atrial arrhythmia was 73.4% by traditional monitoring, with adverse events occurring in only 2.4% of cases.
  • Continuous monitoring revealed that 94% of patients had no arrhythmia episode longer than 24 hours.
  • Arrhythmia burden <0.1% and episodes <1 hour were linked to the lowest healthcare use.
