The combination of semaglutide 2.4 mg and cagrilintide 2.4 mg produced substantial weight loss in adults with type 2 diabetes. The findings from the trial were presented at EASD 2025.
The REDEFINE 2 trial enrolled 1,206 participants with BMI ≥27 kg/m² and HbA1c 7–10%, randomizing them 3:1 to the combination therapy or placebo alongside lifestyle intervention for 68 weeks. Patients receiving the combined therapy lost an average of 15.7% of body weight, compared with 3.1% in the placebo group. Over 50% of participants achieved ≥15% weight reduction, and nearly 30% achieved ≥20%. Glycemic control improved markedly: HbA1c dropped by 2.0% versus 0.1% with placebo, and continuous glucose monitoring showed time in range rose to 89.1% versus 46.1%.
The therapy was generally well tolerated. Gastrointestinal adverse events occurred in 72.5% of participants, mostly mild to moderate. Severe hypoglycemia was rare. These findings highlight the dual benefit of weight reduction and glycemic improvement with semaglutide plus cagrilintide, supporting its role in managing adults with type 2 diabetes and elevated BMI.
