GV increased before and after hypoglycemia in individuals with type 1 diabetes (T1DM). An article published in Diabetology reported an analysis of the Type 1 Diabetes and Exercise Initiative (T1DEXI) dataset examining GV patterns around hypoglycemia in automated insulin delivery (AID) and non-AID users.

The analysis included 498 participants, including 222 AID users and 276 non-AID users. Continuous glucose monitoring trajectories were evaluated from 48 hours before to 48 hours after hypoglycemic events. Glycemic metrics included GV and time below range (TBR). Analyses accounted for exercise, gender, and hypoglycemia duration and severity.

GV increased for up to 48 hours before and after hypoglycemia in both AID and non-AID users. TBR elevation persisted across all groups and peaked around hypoglycemic events. Females using AID showed improved post-hypoglycemia glucose stability compared with non-AID females. This difference was larger than that observed in males. Individual-level analyses showed that hypoglycemia episodes lasting more than 40 minutes were associated with prolonged TBR, including among AID users.

These findings show that GV changes occur over extended periods surrounding hypoglycemia in T1DM. Hypoglycemia duration and gender were associated with differences in post-event glucose stability in both AID and non-AID users.
