A large-scale community-based study published in Archives of Endocrinology Metabolism found that the triglyceride-glucose (TyG) index may be a powerful and practical tool for the prediction of the onset of type 2 diabetes mellitus (T2DM), particularly among young and middle-aged adults, women, and individuals with normal body weight.

The study analyzed baseline data from 8,576 individuals who were followed for five years. They were categorized into two groups: those who developed T2DM (n = 882) and those who did not (n = 7,694).

After adjusting for various confounding factors, the study found that every 1-unit increase in the TyG index tripled the risk of developing T2DM (HR: 3.348; 95% CI: 3.004–3.731; P < 0.001). When participants were divided into three TyG quantiles, individuals in the highest TyG group had a 6.4-fold greater risk of developing T2DM than those in the lowest group.

Subgroup analyses revealed that the TyG index was especially predictive in certain demographics. The association between higher TyG levels and T2DM risk was more significant in younger and middle-aged participants, women, and individuals with a normal body mass index.

Given that both triglyceride and glucose levels are commonly measured in standard blood tests, the TyG index offers a cost-effective and accessible method for identifying individuals at elevated risk for diabetes before symptoms or more severe complications develop.

Key highlights

•    Each 1-unit increase in the TyG index tripled the risk of developing T2DM.
•    The highest TyG quantile is linked to over sixfold increased T2DM risk compared to the lowest.
•    Stronger predictive value observed in young to middle-aged adults, women, and individuals with normal weight.
•    TyG index offers a low-cost, widely accessible biomarker for early diabetes risk assessment.
