A recent study in Diabetes Research and Clinical Practice revealed that, within one year, individuals with combined impaired response to dynamic provocation (CIRDP) and high baseline plus minimal insulin dynamic provocation (HBPMIDP) had a greater diabetes progression than those with stable impaired dynamic provocation (SIDP).
In the Indian ethnic population, only a few data are available on pathophysiological phenotypes for prediabetes. This study aimed to investigate the pathophysiological phenotypes of prediabetes using a data-driven method to find out which one is most likely to be associated with the incidence of diabetes mellitus.
Researchers used 1455 survey samples to characterise the pathophysiological phenotype of prediabetes and its progression to diabetes at one year. Individuals were grouped into different clusters based on their biological data, such as adiposity, blood pressure, dyslipidaemia, glycaemia, insulin resistance, and beta-cell function, using a computer-based algorithm (k-means cluster algorithm).
Researchers identified four different pathophysiological prediabetes phenotypes. Phenotype 1 (n=323), which was Combined Insulin Resistant and Deficient Prediabetes- CIRDP, Phenotype 2 (n=290), which was Severe Insulin Resistant Obese Prediabetes- SIROP, Phenotype 2 (n=544), which was Severe Insulin Deficient Prediabetes- SIDP, and Phenotype 4 (n=298), which was High Blood Pressure Moderate Insulin Deficient Prediabetes- HBPMIDP. Additionally, compared to Phenotype 3- SIDP, the highest odds (OR: 6.07) were seen with Phenotype 1- CIRDP for the progression to diabetes at one year, followed by Phenotype 4- HBPMIDP (OR: 2.05) after adjusting for age, BMI, educational and marital status,
